Payroll Requirements & Documents
- OFC Form 3: Payroll Log Sheet for LPA Projects. Resident Engineer or Town Representative shall use this form to track all work on-site and when payrolls have been received.

Prompt Pay Certifications (Spec 109.09) are due to OAOC: oaoc@dot.nh.gov by the 10th of each month.
